• Your loved one’s Social Security number. This important piece of information allows us to file a Death Certificate.
• Birth Certificate, while not required, your loved one’s birth certificate provides important information for arrangements.
• A photo for memorial cards and/or obituary if applicable
• Military discharge papers (DD-214) so that we can verify their eligibility for veteran benefits with the VA. This document will have the form ID, “DD214” at the lower left corner of the form.
• Clothing for the deceased, even in the event of cremation.
